According to Cincinnati.com Plush called 911 begging "Help, help, help", at 3:16 p.m., but the dispatcher had a hard time figuring out his location.
Kyle Plush, a 16-year-old high schooler, was found dead inside of his 2004 Honda Odessey on the evening of Tuesday, April 10, six hours after calling the police for help. "I'm in the Seven Hills Parking lot, '" the operator said. That information had not been given when he made his first call for help.
In Plush's second call to 911, a banging sound can be heard in the background. But the 911 operators' computers experienced trouble "around that same time frame", one of the internal documents states. At this time, it is not clear if Smith was working overtime when Plush called.
Still, the operator tried sending a text message to Plush, asking him for the address of the emergency.
That operator's supervisors found her work in that incident was "unacceptable", according to one of the internal documents. "He was a young man of keen intelligence, good humor, and great courage, and this whole community feels this loss very deeply".
Kyle: I can't hear you, I'm in desperate need of help.
Around 9 p.m., 911 dispatchers received a call that Kyle had been found in the minivan. Plush was within feet of those coordinates. Officials haven't said exactly where the officers searched.
Plush was reportedly attempting to retrieve tennis equipment from the back seat of his minivan when a row of seats flipped up and over, pinning him upside down beneath them.
After help failed to arrive after the first call, Kyle called 911 again and conveyed that he thought he didn't have much time to live and wanted to make sure his mother knew he loved her.
"This is not a joke", he said.
He was trapped inside a vehicle in a school parking lot. "Send officers immediately. I'm nearly dead".

Teen killed in minivan was trapped under third-row seat, official says
The information released by police Wednesday afternoon did not clarify the reason the first deputy to respond was not able to find Plush's van or where Plush had been inside of it.
"Help, help, help, help", he told the dispatcher, according to 911 audio obtained by The Washington Post.
"He did look into some vehicles".
Authorities said that a classmate called Kyle's parents late Tuesday, saying that he did not show up for a scheduled tennis match. "He never located that".
"We are actively trying to identify experts to assist us in this investigation", Hamilton County, Ohio, Prosecutor Joseph Deters told reporters.
A family member found the teen dead in the vehicle, which was unlocked, an hour later, police said.
Hamilton County Coroner Lakshmi Sammarcho ruled the cause of death as an accidental "asphyxia caused by chest compression", reported WCPO.
An investigation has been launched into what happened.
Chris Carver with the National Emergency Numbers Association said it's rare for a 911 center to experience a serious failure.
Carver and Roger Hixson, the association's technical director, only spoke generally and not on the particulars of this case.
It is unclear if Kyle's van - which was a 2002 model - would have been subject to any recalls. "Help. I need help", he says.
